Professional Disclosure Statement
This is a statement of your rights and responsibilities for our therapeutic relationship. Because you will be putting a significant amount of time, energy, and money into therapy, it makes sense to approach the process of choosing a therapist with selectivity and care. I strongly believe you should feel comfortable with the therapist you choose, and hopeful about the work ahead of you. Research shows that when you feel this way, the probability of therapy being helpful to you is drastically increased. The Professional Disclosure Statement is designed to inform you of my professional credentials, types of service offered, fee schedule, policies, and therapeutic orientation and style. Please let me know if you have any questions or concerns about this disclosure statement.
Counseling Philosophy and Approach
In my view, counseling works most effectively as a collaborative process between the client and therapist. As a registered professional counselor associate, I value your expertise on yourself and your goals because you know yourself best! Since each individual is unique, having their own complex intersections of identity and experience, my theoretical approach is both holistic and systemic - meaning that I believe nothing happens in isolation. Healing happens in community and in relationship with others, and in the context of a trusting and attuned therapeutic relationship. I work with individuals and their families from a Systems lens, and view individuals within the context of their values and beliefs, family and community systems of origin, their current environments, and the larger socio-cultural context. I use an integrative modality informed heavily by Relational-Cultural, Constructivist, Existential, Narrative, Cognitive-Behavioral, and Strength-Based approaches, among others. I also offer relationship counseling and use a variety of tools and approaches such as Emotion-Focused Therapy (EFT) and Internal Family Systems (IFS). I focus on drawing out the strengths of individuals and families that can be used to address presenting problems. Together, we will explore techniques rooted in these theories as well as in the creative arts to strengthen your relationships, tap into your innate strengths and resources, and learn new skills to manage difficult emotions.
Art Therapy
I am trained in the use of art therapy as part of the intake and treatment process, and can offer art therapy interventions that address the goals in your treatment plan. I am provisionally registered with the Art Therapy Credentialing Board (ATCB) and actively working towards certification as an art therapist (registration number: 23-203). Art therapy provides another method of exploring and addressing issues, particularly those that are often difficult to talk about. No previous experience or skills are required in order to engage in and benefit from art therapy activities. I am always happy to discuss incorporating art therapy techniques into treatment if you wish to pursue this method of treatment.
I have completed a certificate program in eating disorders counseling. When it comes to food and weight-related concerns, I approach them from an anti-dieting framework. I understand that eating disorders are complex and uniquely experienced by each person, and I adopt a weight-inclusive and intersectional approach to healing from disordered eating patterns.
I am comfortable working with individuals from diverse cultures and lifestyles. I recognize the impact of socio-cultural systems on members of minority groups. I firmly believe that every individual possesses the ability to solve their own problems, and my role as a therapist is to facilitate the discovery process within the context of your cultural worldview. If there are significant differences in our cultures and belief systems, I am committed to understanding and respecting those differences.
To meet your unique needs and comfort level, I may integrate methods and techniques from various therapeutic approaches. These may include talk therapy, art, mindfulness exercises, journaling, exploring thoughts and behaviors, keeping track of your daily experiences, movement, role play, assigned activities, music, and imagery. Additionally, as a certified yoga teacher through Yoga Alliance (RYT 200, #296477), I may incorporate aspects of yogic philosophy or practice into our sessions, depending on your individual goals and needs.
During our initial consultation and intake process, we will work together to determine which methodologies and techniques will best serve your treatment goals. Your input and preferences are highly valued, and I strive to create a therapy experience that is tailored to your specific needs and aspirations.

I have taken undergraduate and graduate coursework in psychopharmacology, addictions counseling, human growth and development, transgender and nonbinary studies, adolescent art therapy, adolescent and family development, art therapy technique, clinical assessment by graphic means, group counseling, diversity and equity in counseling, ethics in counseling, art therapy research, diagnosis of mental and emotional disorders, trauma treatment in art therapy, Applied Sucide Intervention Skills Training (ASIST), addictions counseling, treatment of eating disorders, nutrition and Health At Every Size (HAES), fat studies, and the mind/body connection in the treatment of eating disorders.
As a Registered Professional Counselor Associate (registration # R8611) and a Qualified Mental Health Professional Registrant (QMHP-R, registration # 23-QMHP-R-2005) in the state of Oregon, I will abide by the rules and Code of Ethics for the Oregon Board of Licensed Professional Counselors and Therapists. Everything you share within the counseling room will be confidential with a few exceptions which include:
As a registered professional counselor associate and QMHP-R, I am required to discuss relevant concerns with my supervisor in service to my learning and to provide you with high quality care. My supervisor is named Quinn Rivenburgh, MAAT, ATR-BC, LAT, LPC. Additionally, I may also discuss pertinent topics with my supervision group, but I will not reveal your name or identifying information. You are always welcome to ask me any questions you may have about this, and I will never reveal your name or identifying information about you to anyone other than my supervisor named above.
If you direct me, in writing, to tell someone else about your care.
If you are determined to be a danger to self or others.
If there is reasonable cause to suspect abuse of a child; which I am legally obligated to report.
If there is reasonable cause to suspect abuse of an at-risk adult or an at-risk elder, which I am legally obligated to report.
If I am ordered by a court to disclose information.
Confidentiality and Treatment of Older Children and Teens
Oregon law allows clients 14 years and older to consent to their mental health treatment, but parents must be involved before treatment ends, except in rare cases. If a minor self-consents, their medical records remain confidential unless disclosure is clinically appropriate, the minor needs detoxification from a substance, is at serious risk, or requires inpatient admission due to a deteriorated condition or imminent risk of suicide.
In Washington, minors 13 years and older can make their own decisions about mental health, substance abuse treatment, and some medical consultations without parental consent. Parents can access certain aspects of their child’s medical records as a “proxy” (such as viewing upcoming appointments, viewing and updating insurance information, etc.) but they cannot view healthcare information related to reproductive and sexual healthcare, mental health care, and substance use care.

At this time, I provide 37-minute sessions, 53-minute sessions, and 75-minute sessions currently. I serve children, adults, couples/partners, families, and groups. I can provide superbills for out of network (OON) benefits, or, for those who have out of network benefits, I can use an app called Thrizer to submit claims out of network on your behalf.

Cancellation Policy
Since the scheduling of an appointment involves reserving time specifically for you, a minimum of 48 hours' notice is required for cancellation to avoid a cancellation fee. However, if frequent cancellations or no-shows become a pattern (i.e., three or more cancellations or no-shows in 3 months), you may be notified of the risk of discontinued treatment. This policy allows for the accommodation of other clients in need of services, as well as ensures the sustainability of the mental health services provided. Additionally, if you are unable to attend an in-person session, you may inquire about the possibility of meeting virtually as an alternative. The fee for late cancellations (less than 48 hours' notice) is set at half the session fee. The fee for no call/no shows (no notice) or less than 24 hours’ notice is equal to the full session fee. If a cancellation can be rescheduled to a different time slot within the same week (depending on clinician availability), no fee will be incurred. Fees may be waived at clinician’s discretion for exceptional and rare circumstances.
Please cancel sessions through the Sessions Health appointment manager or notify me of cancellations via the Sessions Health messenger on the portal. You can also call, text, email, or leave a voicemail.
Frequent late cancellations may result in client discharge from services. After three late cancellations or no-shows, I reserve the right to discharge the client and may place them back on the waitlist for services.
Late Arrivals
If you show up to your appointment over 10 minutes late without first notifying the therapist, I will try to contact you via phone call or text, depending on your stated contact preferences. If I am not able to reach you, I reserve the right to cancel the session and charge a no-show fee. If you are more than 15 minutes late to your appointment, regardless of whether or not you notified me, I may choose to cancel the session. I will then attempt to reschedule the session as availability allows at my discretion. Please be aware that if you are late to an appointment, the appointment will still end on time, and the full session fee will be charged.
Please note that for most clients, insurance will not cover late fees or no-show fees. Clients with OHP/CareOregon insurance are exempt from any fees per regulations, but cancellation and no-show policies still apply.
The Therapeutic Process: Benefits, Risks, and Expectations
The Therapeutic Process
The success of therapy greatly hinges on the client's active participation, which may occasionally involve experiencing significant discomfort. Reflecting on challenging past experiences and delving into the associated emotions may trigger intense feelings such as anger, depression, or anxiety. I am dedicated to standing by clients as they navigate this path toward healing and personal growth. By committing to therapy, clients acknowledge both the potential for discomfort and the opportunity for profound healing and empowerment. It's essential to recognize that specific risks and benefits of therapy are inherent in this transformative journey.
Risks, Benefits, and Expectations
While I may offer tools and skills for change, it is your responsibility to practice the tools and skills. You have the right to refuse any technique or negotiate a modification of any technique that you believe may be inappropriate for you.
As a client, you have the right at any time to discuss the positive and negative effects of counseling with me.
It is important that you are aware of the risks involved in the counseling services offered.
You may experience interruptions in your daily patterns. Your social relationships may enter moments of conflict in order to work through differences. Some clients experience worsening symptoms as they work through them before they get better. You may be frustrated by how quickly or slow the process is moving. You may feel that outside forces are inhibiting your movement forward or are causing disruptions in your life. At times, you may feel out of control of the process.
You will also experience benefits to engaging in counseling. Many people experience a significant increase in the connection, support, and togetherness they feel. You may feel like you are not alone. You may gain insight, information, and resources that help you know your path forward. You may increase the open communication between family members. You may experience relief from pain, worry, sorrow, despair, and hopelessness. You may see hope and potential for your future. You may find an interest in giving back to your community.
The nature of counseling relationships is time-limited. There will be a time when the relationship with your counselor ends, and this may feel premature or may bring up feelings of loss or abandonment. You have the right to end your counseling relationship at any point. Should you and/or I believe an outside referral is needed, referrals will be made to other providers or agencies more appropriate for your needs (for more about this, see the Length of Treatment and Ending Therapy section below).
It is your responsibility to follow up on outside referrals or recommended resources.
You will be best served if the sessions concentrate exclusively on your concerns.
I do not provide my personal telephone number or email address to clients. In case of an emergency, please use the contact information for a 24-hour crisis line and other resources listed in this document.
As with adults, children & youth may experience interruptions in their normal patterns, feelings,
and social/familial relationships during the course of counseling. The following list highlights
common effects or responses to counseling:
They may talk about feelings and thoughts and ask questions regarding their gender identity that they may not have shared with family members before. Discussing these issues may be emotionally stressful for family members, and you are encouraged to process this in your sessions with me.
They may use new words and ideas they have learned in counseling to define behaviors of people around them, including family members. This can be challenging for family members who may feel criticized by their children.
They may behave in a more assertive way (or what seems to them to be assertive). For example, a child may resist unwanted touch while maintaining safe personal space. They may display intensified behaviors (acting out or withdrawn) after counseling/group sessions that were emotionally stressful for them.
It is important that children feel validated and supported (not punished) during these times. We can discuss ways to support your child after difficult sessions.
Treatment Frequency, Length of Treatment, and Ending Therapy
In my practice, I view mental health care as a collaborative journey. Together, we will create a treatment and recovery plan, potentially in consultation with your other healthcare providers. If you ever feel uncertain about your treatment goals or any aspect of therapy, please do not hesitate to discuss it with me.
Frequency and Duration of Therapy
Typically, therapy sessions are scheduled on a weekly or biweekly basis. We will consider ending treatment once your goals are met, if another provider is deemed more suitable, or if it is required by your health plan or determined necessary by me.
Ending Therapy
You have the right to discontinue therapy at any time and for any reason. However, we aim to discuss the conclusion of therapy in advance to ensure a supportive, collaborative, and meaningful ending. Ideally, the conclusion of therapy should take place in person or via telehealth, spanning one to three sessions. If you ever feel that therapy is not aligning with your needs, you have the right to request a change in direction or to discontinue therapy.
If more than 45 days pass without contact from you, it will be considered your indication to terminate our therapeutic relationship. Should you decide to return to therapy at a later time, we will explore the available options together.
Clinician-Initiated Termination
There are circumstances under which I may choose to end therapy. These include, but are not limited to:
Concerns about attendance or frequent tardiness.
Non-payment for services.
A determination that I am not a good fit for your needs.
Situations that arise outside of my scope of practice.
Long-term illness or incapacitation on my part.
Career changes or other professional reasons.
If any of these situations occur, I will provide as much notice as possible and offer closing session(s) to facilitate a smooth transition. I will also provide referrals to other qualified clinicians to ensure continuity of care.
Communications Policies
Telephone Calls and Emergency/Urgent Services
I can be reached at (971)270-0535 during my normally scheduled business hours, between Monday and Thursday from 9am to 5pm. On these days, I check my voicemail once per day in general, and attempt to return all calls within 24 business hours to your inquiry. I do not typically work or return messages on Fridays, Saturdays, or Sundays at this time, with some exceptions for when I am occasionally in office on those days. Be aware that there may be times when I am unable to receive or respond to messages even during my normally scheduled hours, such as when out of cellular range or out of town.
If you are experiencing big emotions, big changes in your life, or need some resources or problem-solving before your next session, you can call me to set up a 10-15 minute phone call, subject to availability. I will typically respond within 24 business hours to your inquiry during business hours.
Email Communications and Texting
Email and/or texting is not generally used as a form of clinical communication. For your privacy and protection, the secure messaging service through the Sessions Health portal will be used for most of our communications. There are some situations in which you and/or your clinician may decide to use email and/or texting for communication, such as for scheduling purposes. Please note that email and/or texting are not secure forms of communication per HIPAA. Though you may send emails or texts at your own risk, I will only be able to reply or initiate clinically appropriate emails if you provide written consent, and will likely respond instead via the Sessions Health portal for requests other than scheduling. Email and text are not appropriate for urgent or emergency communications. I cannot guarantee that any particular email and/or text will be read and responded to within any particular period of time.
If you use your work email to communicate with me, your employer may access our email communications. There may be similar issues involved in school email or other email accounts associated with organizations that you are affiliated with. Additionally, people with access to your computer, mobile phone, and/or other devices may also have access to your email and/or text messages. Please take a moment to contemplate the risks involved if any of these persons were to access the messages we exchange with each other.
Be aware that all communications (i.e., messages, texts, emails, voicemails, files sent, images sent, etc.) may be filed into the client’s medical record. It is important that we be able to communicate and also keep the confidential space that is vital to therapy. Please speak with me about any concerns you have regarding any communications policies.

A dual relationship includes any form of familial, social, financial, business, or close personal relationship with clients, including through social media. There is a possibility we may encounter each other outside of the therapy room. If that should happen, I will never approach you first and will always maintain confidentiality. That said, you are welcome to approach me, although I will refrain from engaging in any lengthy conversations in order to protect your privacy. I will never disclose the nature of our relationship to anyone unless you give me explicit permission in writing to do so. We can discuss the impact and any needs you may have around such an encounter at our next scheduled session should this occur.

Court Testimony
Please understand that in the therapeutic process at Green Sheep, we aim to avoid involvement in legal proceedings against current or former clients and their families. When you engage in treatment with Green Sheep, you agree not to use these services or seek records for legal purposes in cases where marital or family therapy has not successfully resolved disputes. This guideline helps ensure the integrity of the treatment process and prevents misuse for legal objectives.
If you anticipate or are already involved in legal proceedings, kindly inform me promptly, so we can assess together how it may impact our therapeutic work. It's important to note that our therapy is not equivalent to a forensic or custody evaluation. If you require such an evaluation, I can assist you in finding an appropriate provider. If I am subpoenaed to provide testimony, I will do my best to protect your confidentiality; however, there may be limitations, as detailed in the confidentiality section. It's important to be aware that there is a charge for any testimony, covering time spent on preparation, consultation, travel, and record copies. My fees are at the current legal rate of $250 per hour, with a minimum of 4 hours, in addition to expenses for copying and sending records. Please be aware that you are responsible for these fees, as they are typically not covered by insurance.
Contagious Illnesses Statement
The information in this section applies to IN-PERSON sessions, and does not apply to telehealth appointments. If there is a resurgence of the pandemic or if other health concerns arise, I may require that we meet via telehealth. If you have concerns about meeting through telehealth, we can talk about it first and try to address any issues. If I believe it is necessary, I may determine a return to telehealth for everyone’s wellbeing.
Please do NOT come to session if you are experiencing any symptoms of ANY contagious or potentially contagious viral illness.
For in-person appointments, masks are optional at this time, but this may change depending on my perceived level of risk. If you appear to have symptoms of a contagious or potentially contagious illness - even if you are wearing a mask - I may stop the session and reschedule at my own discretion. Please wash your hands or use an alcohol-based sanitizer before attending in-person sessions. If you decide at any time that you would feel safer staying with, or returning to, telehealth services, I will support and respect that decision as long as it is feasible and clinically appropriate.
If you (or your therapist) test positive for COVID or contract another potentially contagious illness, my policy is to move to telehealth for a period of two weeks (typically two sessions) before resuming in-person services.
Telehealth-Specific Policies
Privacy and Confidentiality
To ensure client safety, privacy, and confidentiality, telehealth sessions must take place in a private location where you will not be overheard or interrupted. It is crucial to choose a quiet, secure environment to maintain the confidentiality of our sessions. If it is determined that you are not in a private location or are likely to be overheard by others, we will end and reschedule the session. This policy is in place to ensure your safety and the confidentiality of our therapeutic work.
Prohibition of Driving During Sessions
For your safety and the security of our communication, telehealth sessions cannot be conducted while you are driving. If it is determined that you are driving during the session, I will end the session immediately. This policy is in place to ensure your safety and the confidentiality of our therapeutic work.
Session Etiquette
Telehealth sessions should be treated with the same respect and formality as in-person sessions. Clients are expected to be fully clothed and present themselves appropriately during telehealth appointments. This ensures a professional and respectful therapeutic environment.
Technical Requirements
You will need a reliable internet connection to ensure a smooth and uninterrupted session.
A device with a functioning camera and microphone is required for video sessions, preferably a desktop with a camera, a laptop, or a tablet. Make sure your device is fully charged or plugged in during our session.
It is recommended to use headphones to enhance audio quality and ensure privacy.
Backup Plan for Technical Issues
In case of technical difficulties, we will have a backup plan to continue our session. If video conferencing fails, we may switch to a phone call. I will attempt call you if our connection is lost.
Emergency Protocols
Please provide your current location at the start of each telehealth session if we are meeting over telehealth for the first time, or if you are conferencing from a new location. This information is necessary in case of an emergency where immediate assistance may be required.
Environment and Distractions
To maximize the effectiveness of telehealth sessions, try to minimize distractions. This includes turning off notifications on your device, informing household members of your need for privacy, and avoiding multitasking during the session. Please refrain from texting or engaging in other activities during our session to maintain focus and the quality of our therapeutic work.

Legal and Ethical Considerations
Telehealth services will be provided in accordance with state laws and professional ethical guidelines. I am able to provide telehealth services only in the state(s) where I am registered to practice. Please ensure you are physically located within these states during our sessions.
By adhering to these telehealth-specific policies, we can ensure a safe, confidential, and effective therapeutic experience. If you have any questions or concerns about these policies, please feel free to discuss them with me.

Client Rights and Responsibilities
As a client, you have the right to choose a counselor/therapist who best suits your needs and purposes. Please be advised that you may ask questions about treatment at any time, and you may also choose to terminate/end therapy at any time. As a client of an Oregon Registered Associate, you have the following rights, as defined by rule of law [OAR 833-100-0021 (14)]:
To expect that a registered associate has met the qualifications of training and experience required by
state law;To examine public records maintained by the Board and to have the Board confirm credentials of a registered associate;
To obtain a copy of the Code of Ethics;
To report complaints to the Board;
To be informed of the cost of professional services before receiving the services;
To be assured of privacy and confidentiality while receiving services as defined by rule or law, with the following exceptions: 1) Reporting suspected child abuse; 2) Reporting imminent danger to you or others; 3) Reporting information required in court proceedings or by your insurance company, or other relevant agencies; 4) Providing information concerning associate case consultation or supervision; and 5) Defending claims brought by you against me; and
To be free from being the object of discrimination on any basis listed in the Code of Ethics while receiving services.
